The ambulance division of MediRide, Inc. has now become a Blue Cross/Blue Shield of Michigan (BCBSM)
Provider. MediRide would like its patients to know if their ambulance transport with us will be covered by BCBSM.

Determining Eligibility
To determine if you have ambulance service benefits, locate the professional-service code on your BCBSM I.D.
card. Find the second set of four characters in the COVERAGE CODE. If you see the abbreviations "AS" or "AS-1"
that means that your contract has ambulance service benefits that can be utilized by MediRide, Inc. or other
licensed ambulance operators. If you DO NOT have either an "AS" or "AS-1" code listed, you may still qualify if
your contract has Master Medical coverage for ambulance service benefits.

Ambulance Services
Ambulance services are transportation and life-support services furnished to sick, injured or incapacitated
patients by a licensed ambulance company. BCBSM pays for three types of ambulance services including the
BLS service offered by MediRide, Inc. Not all contracts provide the same level of ambulance benefits. Contact
BCBSM to inquire further about your benefits.

General Requirements
Ambulance services must be medically necessary. Ambulance services are not considered medically necessary
if any other means of transportation could be used without endangering the patient's health.

Patients must be transported in an ambulance vehicle licensed by the State of Michigan as an ambulance, and
the ambulance must carry personnel qualified to treat the patient.

Ambulance services are payable only when provide for the following:
*Transporting a patient to a hospital
*Transferring a patient from a hospital to another treatment location such as another hospital,
skilled nursing facility, a medical clinic or the patient's home (the attending physician must
order the transfer).

General Limitations & Exclusions
BCBSM does not pay for the following:
*Ambulance trips that are not medically necessary
*Use of vehicles not certified by the State of Michigan as an ambulance
*Separate charges for nurses or other nonmandatory personnel, as defined by state law
*Separate physician charges for riding with a patient
*Services of fire departments, rescue squads or other plans whose fees are paid by voluntary
donations
*Ambulance trips when a patient is not transported
*Ambulance services for the convenience of the patient, family or physician

If you are utilizing your Medicare benefits as your primary payor source, your claim will be forwarded to BCBSM for
you by your Medicare contracted carrier.
